The Maddox Foundation, a Mississippi nonprofit corporation, Employer Identification Number 64-0917900, was issued a determination of status as a tax-exempt organization as described in Section 501(c)(3) of the Internal Revenue Code and as a private foundation within the meaning of Section 509(a) of the code, on March 2, 2000.
